To request a replacement permanent ID card, please call customer service at HealthEZ, 952-896-9130 or 800-203-1514. You can also print your own temporary ID card by logging in and clicking the "Your ID Card" link in the left menu.
Please visit Caremark.com, the web site for your pharmacy benefit manager (PBM). You can use the subscriber ID on your health plan ID card to register, and then you'll be able to refill prescriptions, find a pharmacy, etc.
